Terms & Conditions

1. Definitions

"Practice," "DermoVox," "we," "our," and "us" refer to DermoVox LLC and its affiliated healthcare providers, contractors, employees, and authorized representatives.

"Patient," "Client," "you," and "your" refer to the individual receiving healthcare services.

"Services" include, but are not limited to:

  • Medical Weight Management

  • Metabolic Health Services

  • Hormonal Wellness Services

  • Endocrine Wellness Services

  • Virtual Sick Visits

  • Preventive Wellness Services

  • Physical Examinations

  • Occupational Health Services

  • Laboratory Review

  • Medication Management

  • Telehealth Consultations

  • Additional healthcare services offered by DermoVox from time to time within the provider's licensure and scope of practice.

2. Scope of Services

DermoVox provides non-emergency healthcare services through virtual and in-person appointments.

Services may include evaluation, diagnosis, treatment recommendations, medication management, laboratory review, preventive care, wellness counseling, physical examinations, and occupational health evaluations when clinically appropriate.

Healthcare recommendations are individualized and may vary based on your medical history, examination findings, laboratory results, and clinical judgment.

Nothing on this website guarantees a diagnosis, prescription, treatment plan, medication approval, or medical clearance.

3. Telehealth Services

By scheduling a virtual appointment, you acknowledge that:

  • You are physically located in a state where your provider is licensed to practice at the time of your visit.

  • Telehealth may not be appropriate for every medical condition.

  • Certain services require an in-person examination.

  • Technology interruptions may require rescheduling.

  • DermoVox may recommend emergency evaluation, urgent care, specialty referral, or in-person follow-up when clinically indicated.

4. No Emergency Services

DermoVox does not provide emergency medical care.

If you experience chest pain, difficulty breathing, stroke symptoms, severe bleeding, suicidal thoughts, or another medical emergency:

Call 911 immediately or go to the nearest emergency department.

Patient portal messages, emails, phone calls, and text messages are not monitored continuously and should never be used for emergencies.

5. Appointments and Payment

Payment is required before services are rendered unless otherwise agreed.

Appointment fees vary depending on the service provided.

Memberships, subscription programs, and package pricing will be governed by their individual program agreements.

Missed appointments, late cancellations, and no-shows may be subject to cancellation fees.

6. Medication Prescribing

Prescribing medication is never guaranteed.

Medications are prescribed only when medically appropriate based on:

  • Clinical evaluation

  • Medical history

  • Current medications

  • Laboratory findings

  • State laws

  • Professional standards of care

Certain medications require ongoing follow-up appointments and laboratory monitoring.

7. Pharmacy Services

When medication is prescribed, prescriptions may be transmitted electronically to:

  • the patient's preferred pharmacy,

  • a mail-order pharmacy,

  • or an approved pharmacy partner.

Medication availability, pricing, shipping times, shortages, delays, and fulfillment are solely the responsibility of the dispensing pharmacy.

DermoVox does not manufacture, compound, dispense, or ship medications.

Medication purchases are generally non-refundable.

8. Laboratory Testing

Some services require laboratory testing before treatment or during ongoing care.

Failure to complete requested laboratory testing may delay treatment or prevent medication prescribing.

Laboratory fees are separate unless specifically included in a program.

9. Physical Examinations

Physical examinations are performed for wellness, employment, school, sports, occupational health, or other approved purposes.

Scheduling a physical examination does not guarantee:

  • medical clearance,

  • employment clearance,

  • sports participation clearance,

  • fit-for-duty approval,

  • DOT certification,

  • or completion of requested forms.

Medical decisions are based solely on examination findings and applicable clinical standards.

10. Occupational Health Services

Occupational health evaluations are performed according to employer requirements and professional medical judgment.

DermoVox does not make hiring, employment, disability, workers' compensation, or return-to-work decisions.

Employers remain responsible for employment determinations.

11. Wellness Programs

Wellness services are designed to promote overall health and disease prevention.

Individual results vary.

No guarantees are made regarding:

  • weight loss,

  • metabolic improvement,

  • hormone optimization,

  • laboratory outcomes,

  • symptom resolution,

  • or treatment success.

12. Insurance

DermoVox operates primarily as a cash-pay practice.

Unless otherwise stated, services are not billed to Medicare, Medicaid, Tricare, or commercial insurance.

Patients may request an itemized receipt for possible out-of-network reimbursement.

13. Communications

Patients are encouraged to use the secure patient portal for non-urgent communications.

Text messaging, email, and voicemail are not guaranteed to be secure or continuously monitored.

14. Privacy

DermoVox protects patient information in accordance with applicable federal and state privacy laws, including HIPAA where applicable.

15. Cancellations and Refunds

Refund eligibility varies by service.

Professional consultation fees are generally non-refundable once services have been rendered.

Medication costs, laboratory fees, pharmacy charges, and shipping fees are non-refundable.

16. Limitation of Liability

DermoVox shall not be liable for delays or failures resulting from:

  • pharmacy shortages

  • medication backorders

  • shipping delays

  • laboratory delays

  • internet outages

  • technology failures

  • events outside our reasonable control.

17. Governing Law

These Terms shall be governed by the laws of the State of Ohio except where another state's laws are required because healthcare services are provided in that jurisdiction.

18. Changes to These Terms

DermoVox reserves the right to update these Terms and Conditions at any time.

The most current version will always be posted on our website.

Continued use of our services constitutes acceptance of any revisions.

19. Entire Agreement

These Terms and Conditions, together with our Privacy Policy, Informed Consent documents, Financial Policy, and any applicable program agreements, constitute the entire agreement between the patient and DermoVox.
